This consent applies to Hyper-Realistic Nano Brows, Soft Lip Blush and PMU touch-ups at KAD Brows. By booking one of these services, I confirm that:

• The health information I provide in my booking form is true and complete, and I will tell KAD Brows about any change before my appointment.

• I understand that results may vary depending on my skin type, healing and aftercare.

• I understand there may be temporary redness, swelling or discomfort after the procedure.

• I understand that additional sessions, such as a touch-up, may be needed for best results.

• I will follow the aftercare instructions I receive after my appointment.

• I confirm that I will not be under the influence of drugs or alcohol at my appointment.
